Bloomhaven Concierge Medicine and Wellness is a mobile primary care, urgent acute care, psychiatric, and diagnostic medical practice operating in Greater Houston, Texas. We operate on a door to door dispatch model, delivering non emergency care directly to private residences, senior communities, and institutional campuses.
You are seen by a licensed clinical professional, such as a nurse practitioner, registered nurse, LVN, medical assistant, or patient care technician. Our physician directs the care team and reviews your history, medications, and results over time.
We are a mobile, in person, preventive care practice. Visits happen at your door, at home, in senior communities, and on institutional campuses across Greater Houston, where we check vitals, draw specimens, and run testing in person. Telehealth is reserved for patients we have already seen.

Clinical Care Pathways
Every pathway begins with a mobile visit from a board certified provider. Diagnostics, mobile ultrasound, nutrition therapy, and medication reviews are ordered and interpreted during that house call.

Bloomhaven brings mobile, in person care to your campus. When a child is not feeling well, our licensed clinicians come to the school, evaluate on site, and send the parent the complete care report the same visit.
On site sick visits and rapid testing for strep, flu, RSV, COVID 19, and UTI.
Consent and insurance collected once at enrollment, so the visit is ready when a child is not feeling well.
The full care report and care instructions emailed to the parent right after the evaluation.
